Prestige Staffing is looking for a Hospice Case Manager who travels to people's home to review patient care.
This position could be temporary, temp-to-perm or permanent.
Excellent benefits when permanent .
Will have rotating on call for nights and weekends.
$35 - $37 plus mileage

The Case manager oversees and directs all clinical aspects of patient care, assuring quality patient care through professional assessment, intervention, evaluation, and supervision of the nursing process and nursing plan of care.


Must live in or near Milledgeville, GA

  • Currently registered in Georgia and in good standing
  • 3+ years nursing experience providing direct patient care
  • 2+ years hospice experience and demonstrated ability to manage all aspect of terminal care
  • Excellent clinical skills involving, but not limited to, such aspects as intravenous therapy, nutrition support, pain management, central line management, and nursing assessment
  • Valid driver's license and proof of insurance for the vehicle driving for work daily


  • Assures that all clinical services provided are consistent with hospice standards of practice and policies and procedures.
  • Serves as clinical resource for patient problems and issues. Coordinates all aspects of healthcare for patient as deemed medically necessary.
  • Orders all pharmaceutical and DME supplies and verifies supplies billing.
  • Oversees and supervises all nursing services provided by Hospice as a means of ensuring maximum efficiency, quality, and professionalism.
  • Serves as consultant to other professionals by providing clinical expertise in areas of hospice care.
  • Coordinates and leads Interdisciplinary Team meetings for the development, review and revision of the patient/family plan of care.
  • Provides direct patient care based on staffing requirements and/or for the purpose of clinical supervision.
  • Maintains regular contact with physicians and other medical personnel as a means of communicating changes in patient status, taking medical orders, and ensuring continuity of patient care.
  • Participates in on call rotation as necessary, including weekends and designated holidays.
  • Assists with collection and evaluation of data relating to QA indicators and develops corrective action plan.
  • Makes visits to hospice home care patients as ordered in plan of care to provide clinical expertise and assessment.
