One of our clients is looking for a Primary Care Provider to join their team. You will play a crucial role in providing comprehensive care to patients in long-term care facilities. Responsibilities:
- Provide comprehensive healthcare services, including assessments, diagnoses, and treatment plans.
- Collaborate with a team of healthcare professionals for seamless patient care coordination.
- Order and interpret diagnostic tests, prescribe medications, and administer treatments.
- Offer counseling to residents and their families, focusing on person-centered care.
- Manage chronic conditions, monitor progress, and adjust care plans as needed.
Requirements:
- Nurse Practitioner or Physician Assistant License
- DEA License
