Intake Coordinator
Job Title: Intake Coordinator
Job Summary: We are seeking a detail-oriented and proactive Intake Coordinator to join our dynamic care teams. This role is essential in managing a high volume of incoming referrals and facilitating smooth patient onboarding. You will support our dedicated teams by handling intake processes and coordinating client information efficiently.
Key Responsibilities:
- Manage and process incoming referrals and patient intake, ensuring timely and accurate information capture.
- Collaborate with care teams, including doctors, PAs, and clinical assistants, to streamline the intake process and address any backlog of referrals.
- Utilize EMR systems (Allscripts/Veradigm) and their Intake System (Phreesia) for managing patient information and processing documentation.
- Handle demographic data, schedule appointments, and coordinate with clinical assistants on administrative and clinical tasks.
- Support the transition and onboarding of patients, ensuring a seamless integration into the care system.
Qualifications:
- High school diploma or equivalent; relevant experience in healthcare administration or a related field is preferred.
- Experience as a Medical Assistant is helpful
- Experience in intake coordination, preferably with knowledge of infusion and neurology or a strong foundation in primary care.
- Proficiency with EMR systems, specifically Allscripts/Veradigm, and experience with Phreesia.
- Strong organizational and multitasking skills, with an ability to manage a high volume of referrals efficiently.
- Excellent communication skills and a collaborative approach to working with care teams.
Additional Skills:
- Clinical knowledge or experience, particularly in areas relevant to intake processes, is a plus.
- Ability to handle administrative tasks with accuracy and efficiency while maintaining a high level of patient care.
- Flexibility and adaptability to work effectively in a transitioning environment.
