Patient Care Coordinator
Pay: $20-24/hour DOE
Location: On-site at Clifton Rd., Atlanta, GA
Summary:
Seeking a dedicated Patient Care Coordinator to support outpatient surgical operations through efficient care coordination, appointment scheduling, and exceptional patient service. The ideal candidate will be committed to enhancing the patient experience by serving as the primary point of contact and liaising closely with clinical teams to ensure seamless care delivery.
Requirements:
- High school diploma or GED required; college degree a plus
- Minimum of 3 years of recent experience in patient care coordination or surgery scheduling
- Proficient in computer skills, with experience using EPIC preferred
- Knowledge of medical terminology
- Excellent verbal and written communication skills
- Ability to deliver high-quality customer service
- Able to pass drug screening and background check
Responsibilities:
- Serve as the primary point of contact for established patients and their families, providing administrative support and care coordination
- Schedule surgeries, appointments, and ancillary services in a timely and organized manner
- Collaborate with physicians and clinical staff to triage, coordinate, and manage patient care appointments
- Respond to patient inquiries, offering emotional support and resolving administrative issues
- Ensure all care episodes are coordinated smoothly, enhancing patient convenience and satisfaction
- Communicate effectively with staff, providing feedback and participating in team activities to improve workflow
- Maintain communication with patients to keep them informed and involved throughout their care journey
