Case Manager
Job Title: Case Manager
Pay: $21.00 - $24.50 per hour
Location: Hybrid (Community setting with some remote work)
Summary:
Join a dedicated team committed to supporting individuals in achieving housing stability through comprehensive case management and psychosocial support. As a Housing Support Case Manager, you will play a vital role in assisting clients with lease processes, move-in logistics, rent payments, and navigating landlord-tenant relationships. This role offers an engaging opportunity to make a meaningful impact in a rewarding, flexible environment, managing your own schedule while contributing to a vital community program. The team works with approximately 500 clients, focusing on proactive support to ensure ongoing housing stability.
Requirements:
- Bachelor's degree in human services, psychology, counseling, criminal justice, sociology, social work, or related field, or equivalent education and experience
- Minimum of 1 year experience in behavioral health, case management, or community support services
- Strong care coordination skills and proficiency with electronic medical records and telehealth tools
- Excellent interpersonal and communication skills
- Valid Georgia driver’s license, reliable transportation, and ability to travel throughout Region 3
- Ability to pass pre-employment drug screening and background check
- Strong organizational skills and ability to manage a caseload effectively
Responsibilities:
- Manage a caseload of 25-30 clients, conducting weekly visits in the first three months, then twice a month thereafter
- Assist clients with housing search, lease signing, move-in coordination, and furnishing needs
- Support clients in rent payments, lease renewals, and addressing landlord or neighbor issues
- Develop and implement Housing Stability Support Plans tailored to individual needs
- Conduct safety and wellness checks, housing safety inspections, and early intervention to prevent lease violations or housing disruptions
- Facilitate connections to community services, internal resources, and external providers
- Assist with annual housing recertifications, inspections, and voucher conversions
- Document all service contacts accurately and timely (within 24 hours)
- Participate in team meetings, provide input on client needs, and meet productivity standards (40 billable hours/month)
- Engage in ongoing professional development and supervision sessions
- Perform additional duties as assigned by leadership
